AMERICANS STILL NOT EATING ENOUGH FRUITS AND VEGETABLES

Despite mom"s good advice, most of us aren"t eating our fruits and vegetables, at least not enough of them, according to a large recently released government study.

Only about 27 percent of adults in the USA ate vegetables three or more times a day in 2005, and 33percent ate fruit two or more times a day that year. A higher percentage of women than men ate this much, according to interviews with more than 305,000 people conducted for the Centers for Disease Control and Prevention (CDC). Research shows that diets high in these foods decrease the risk of heart disease, cancer, diabetes and help with weight control. For years, the government"s dietary guidelines have recommended that people eat at least two servings of fruits and at least three servings of vegetables daily. Now the guidelines are more specific, suggesting consumption of at least 2 cups of fruit and 2 1/2 cups of vegetables a day for someone eating about 2,000 calories.

Other findings from the Behavioral Risk Factor Surveillance System:
  • College graduates have the highest level of vegetable intake.
  • Hispanics have the highest intake of fruit consumption and lowest intake of vegetables.
  • Whites have the highest intake of vegetables.


The CDC"s Larry Cohen says people may not be eating these nutrient-rich foods because of cost, lack of time for preparation or lack of easy access to them.


To get people to eat more of these nutrient-rich foods, the CDC, the Produce for Better Health Foundation and other leading health groups are launching a new initiative Monday called "Fruits & Veggies � More Matters." This will replace the "5 A Day" campaign. This will be a "rallying cry" to get people to eat more fruits and vegetables, says Elizabeth Pivonka, president of the Produce for Better Health Foundation. She says the group"s research with moms shows that they know their families should be consuming more fruits and veggies, but they don"t know how to work more into their diets. "They told us that they didn"t want to be lectured to or made to feel guilty or scared into eating more fruits and vegetables. They wanted help and ideas on how to do what they already know that they should do."

While fad diets, diet pills, and fasting may indeed induce rapid weight loss, these methods can cause you to lose muscle and may also injure your heart and other organs fairly quickly. The best solution? Don"t go for an overnight miracle. Instead, follow these steps to lose fat rapidly, healthily, and sustainably.

  1. Determine your daily calorie intake . Losing weight is simply a matter of expending more calories than you take in, through exercise and your daily activities. To win the battle, it helps to know how many calories you are consuming in a day.
    • Write down all the things you eat on a typical day. Carry a small notebook with you and write down every snack, every drink, and the contents of every meal. There are also great websites that you can use to keep track of calories, get recipes, and help achieve your goal. Don"t forget to include the pats of butter or the spoonful of sugar in your coffee. It"s best to do this for at least a couple weekdays and a weekend; it"s even better if you can go a full week. There are also calorie tracking websites that can help you to do this, for example the US government website, My Pyramid Tracker.
    • Do an itemized calorie count. When possible, write down the number of calories in each thing you eat as you eat it. Keep in mind that the recommended serving size is often considerably smaller than the serving you actually eat. Look up the calorie count on the internet for foods that don"t have calories listed on the container or for fast food meals. You don"t have to be 100% accurate, but you do want a good estimate of the number of calories you"re taking in. There is an idea that multiplying your own weight by ten will produce a rough estimate of the number of calories you need to eat per day to maintain your weight. This is NOT true, you may wind up grossly under (or perhaps over) estimating the number of calories you should be eating. Use a scientific or health website to determine the number of calories you should eat a day or consult your doctor. Everyone has different metabolisms and there is no blanket rule that covers everyone"s recommended calorie intake. Reducing 500 calories per day from the calories you eat to maintain your weight can help you lose a pound of fat per week.[2]
  2. Go over the list and decide which foods to cut out or reduce. Cutting calories is usually a lot easier than you might think. For example, that daily tall latte in the morning may pack 500 calories. Since a pound of flab (lost or gained) is roughly equivalent to 3,500 calories, replacing that rich beverage with black coffee can help you lose a pound a week. Other easy cuts include salad dressing (salad dressing is the number one source of fat in the average American woman"s diet) soda pop, candy, and butter. Look at the nutritional information for the foods you eat, pay special attention to your intake of saturated fats and empty calories (high-sugar foods). You don"t need to cut these things out entirely, but if you reduce your intake of high-fat, high-calorie foods you"ll lose weight faster.
  3. Seek out alternatives to the unhealthy foods you"ve identified. You can simply reduce the amount of soda you drink or mayonnaise you put on your sandwiches, or you can substitute healthier choices. Drink water instead of soda, for example, or use mustard instead of mayo. Low-fat and low-calorie options are also available for most foods, and many of these are natural, (although some are made with strange chemicals), and tasty. Start trying to eat healthy in most meals:
    • Choose lean meats. Chicken and fish are both very low in fat (and certain fish like salmon, sardines, and fresh tuna are an excellent source of antioxidants, which are also beneficial to your health), so aim to replace some or all of the beef or pork in your diet with these foods.
    • Replace high-calorie side dishes with healthier alternatives. Many people get a ton of calories from side dishes such as macaroni and cheese, French fries, or potato salad. You can eat healthier and lose weight by replacing these with fresh vegetables and salads. Pre-made salads are practically effortless, and when accompanied by a reduced-calorie dressing or no dressing at all, they"re weight-loss gold.
    • Start your days off right. A fattening breakfast of bacon and eggs or a pastry can be replaced with yogurt, oatmeal, high-fiber, low-sugar cereals; or fresh fruit for fruit smoothies. However, for those on a low carb diet bacon and eggs are a great combo for breakfast, since neither have carbs. But don"t fall into the trap of skipping breakfast.   4. Plan your meals. Look for healthy, delicious meals online or in your cookbooks, and create a menu for the week. Make sure that your meal plan reduces your total calorie intake: you"re not going to lose weight if you consume the same amount of calories by eating different foods.   5. Watch your portion sizes. Opening a bag of rice cakes and eating all of them in one sitting isn"t going to help you lose weight. When eating chips, nuts, or dried fruit put a portion in a small bowl and then put the bag away. That way you won"t mindlessly eat a larger portion than you had intended. Even if you only make minimal changes to your diet choices, reducing portion size will inevitably reduce caloric intake. Pick snacks that are low in calories and fat and high in fiber (dried apricots, nuts, rice cakes, fruits, baby carrots, cherry tomatoes, and so on). Vegetables are generally very low in calories, very high in fiber, and full of flavor and nutrition. Avoid starchy vegetables like potatoes, and try to eat vegetables plain, without fatty dressings or dips. Fruit also makes a good snack. Fruit contains more soluble fiber than vegetables, which slows your body"s absorption of carbohydrates, thus releasing energy more slowly, (preventing sugar highs), and keeping you feeling full longer. Fruit juices are not a replacement for the real thing. You need that fiber, and juices often have more calories than the equivalent serving of fruit -- as many calories as soda! Be careful with dried fruits, because without the water, you tend to eat more, and fruits, when dried, are calorie-rich per ounce. With any dry or dried food, be sure to drink plenty of water.
  7. Get more fiber. There are many myths about fiber, but there is science to back up its helpful role in the diet. Fiber keeps the right amount of water in your intestines, making your digestive system work more efficiently and helping to keep you regular. Thus, just eating enough fiber may help you feel slimmer in just a day or so. There is also evidence that fiber in the diet can help prevent stroke and heart disease, ease the effects of diabetes, and may even directly help in weight loss.
  8. Drink plenty of water. Adequate water is essential for health, and a great many people simply don"t get enough. What"s more, if you"re chronically dehydrated, your body will retain water in unflattering places, so if you make sure to get plenty of fluids you can start visibly trimming down in as little as a day.   9. Exercise. Remember, you can lose weight either by decreasing your calorie intake or increasing the number of calories you burn. Any health strategy should include both, but if you want to lose weight fast, exercise is essential.

Moderate aerobic workouts incorporating jogging, brisk walking, cycling, aerobic machines, or classes not only burn calories -- they also keep your heart healthy. Studies have shown consistent weight training to raise the body"s metabolism by 15%. This means an average woman might burn 200-300 more calories at rest every day! Resistance training is the gift that keeps on giving! Although, keep in mind that muscle mass weighs more than fat (don"t be surprised if you gain weight but look slimmer).
  12. Rest properly. This means not only taking at least 24-48 hours between strength training the same muscles, (and taking 1-2 days off from exercising each week); it also means getting enough sleep at night, since sleep deficiency impairs your ability to lose fat.
  13. Be realistic. Don"t expect a miracle. Healthy weight loss can be achieved fairly quickly, but you"ll need to be patient. In addition, be sure to set realistic goals. Make sure that the weight you"re trying to reach is a healthy weight for you, and keeping in mind that gaining pounds of muscle will help you lose fat, be trimmer, and look better even though you don"t actually lose any weight. Your goal should be a healthy body, not a number of pounds! Everyone looks good at a different weight. A short person may look really good at about 112 pounds, but a person of a taller height would just look unhealthy. Keep yourself at a weight that makes you look good, not at a number that sounds good.
  14. Make adjustments. A successful weight loss strategy based on reducing calorie intake and increasing activity can be adjusted to maintain your desired weight once you reach it. Unlike typical diets, this method is sustainable -- it is a lifestyle change, not a binge-and-purge exercise. Slowly adjust your diet and exercise to include more weight training and calories, as much as is comfortable for you. If you do gain any weight back, you want that weight to be lean, toned muscle, not fat. In addition, weight training, no matter what your age, prevents muscle atrophy and can help stave off osteoporosis.
    • Instead of thinking of it as losing a certain amount of pounds, think of it as being a certain weight. So if you are 145 pounds, and you would like to lose 10 pounds, think of it as being 135 pounds. This helps you think about it as a long-term goal and not just losing the weight to gain it back again.
  15. Be confident. You need to believe in yourself! If you want to diet, and you know that it will make you feel better, then you need confidence. Otherwise, the temptation to cheat on your diet will make it harder, and you will not feel better when you do lose weight. Avoid temptations like chocolate, ice cream, and cookies. They may taste good, but there are other foods that taste delicious, too, without being unhealthy. You need to always encourage yourself to achieve the goals that you have set. Learn to evaluate your efforts fairly and objectively. If you fail to achieve your target for the week, find out why is it so. Is it because you have missed a jogging session? Or you have been eating junk food for one of the days? After evaluating, look ahead to next week and try your best to stick to your plan.[4]

  • Determine your objectives to lose weight. To succeed in doing something, you need to have a very compelling objective or reason so that you will not lose track of your goal. Losing weight just for vanity"s sake is usually less helpful than losing weight to improve health. Other than just exercising, you need to have a healthy diet so that you will have a better body and more energy to be positive throughout the day.[4]
  • It"s very good to do a blood test before you begin your new diet and exercise regimen. Some people may not need to lose a lot of weight, but they may need to lower the blood sugar level slightly. You can also check for mineral deficiencies.
  • The goal is to lose fat, not just weight in general. If you are doing strength training during your weight loss (which you should be), you might actually see an initial gain in weight -- don"t worry! You have probably lost fat and gained muscle, which is denser than fat and is much healthier.
    • A better measurement than what a scale reads is body fat percent. If you don"t have access to calipers or a body fat scale, a better gauge is how your clothes fit and how you look rather than your absolute weight. If you can, though, it is good to measure your body fat about once per month. Men should be between 15%-25%, but no lower than 4%; and women 20%-30%, but no lower than 15%. Females have higher fat percentage due to pregnancy and menstrual requirements.
    • Generally, you want to - with a combination of calorie reduction and increased activity - reduce and burn a certain number of calories a day, causing a net loss of calories so your body is forced to use its own stores of fat. How much aerobic exercise you need to perform depends on what you eat, how fast you"d like to lose weight, and how much muscle you gain. A pound of fat contains 3500 calories. Men can safely lose around two pounds a week, and women can safely lose around one pound a week. Any more than this and you might be losing more muscle mass than fat. Thus, with a combination of calorie restriction and aerobic exercise, a man should aim to run a 3500-7000 calorie deficit each week, and women should aim for a 1750-3500 calorie deficit.
  • With regard to weights, men and women differ on their goals with weight training. Women, don"t worry, no amount of weight training will make you as bulky as a man; women just don"t have enough testosterone to build large muscles. The female body-builders you see are either taking steroids or training 2-3 hours every day. Male and female beginners should start with 3 sets of 8-12 repetitions, and should use the heaviest weight you can while still maintaining good form, even to the last repetition of the last set. Get a weight training book or find a reputable website that will show you different exercises and the correct form. When lifting properly, you will stretch the muscle during the exercise, but you should still stretch properly to prevent injury!
  • No matter what the magazine article says, spot improving is a myth! When your body burns fat, it takes it from all over your body, even if all you do are crunches. No amount of crunches will get rid of that gut if they are not backed up by proper aerobic exercise and diet! Washboard abs do not come from millions of sit ups; they come from having low levels of body fat. So if that is your goal, make sure you do plenty of aerobic exercise in addition to your ab routine.
  • Sugar comes in many disguises. Check your food labels: fructose, glucose, molasses, and corn syrup are all sugars. Limit all sugars, but especially stay away from high-fructose corn syrup. It is just a corn syrup full of sugar. Choosing foods without it will help you reach your weight loss goals faster.
  • Remember your body needs "fuel" just like a car needs "fuel". Your body needs the "right" kinds of food to lose weight effectively.
  • Don"t taunt yourself by not letting yourself eat the occasional snack that you"re craving. Just make sure you burn it off later with exercise
  • Despite popular belief it is not terrible to eat after 6 p.m. A reasonable dinner (with extra protein if you"ve had a good weight training session) is going to help deliver nutrients to the body to prepare it for the next day. Additionally if you find yourself very hungry before bed, a small bowl of high fiber cereal and low fat milk will actually help maintain your metabolism while you sleep. The key is to not overeat in the evenings.
  • Take a multivitamin. While it is better to always get your nutrients from your food, most people miss something in their diet, especially when they"re trying to cut calories. Vitamins and minerals play an important role in keeping your body chemistry balanced and your energy levels healthy.
  • If you find yourself missing some of your favorite foods, don"t worry. There is no harm in having your treat once a week. Pick a day each week where you treat yourself to your guilty pleasure--remember, everything in moderation. If it"s pizza, try going to a gourmet pizza place that will use better ingredients and fresher vegetables. If it"s a Big Mac, try making your own. A homemade burger is generally much healthier than the food they serve in fast food restaurants. Electric grills that are slanted to allow most of the fat to slide off make nice, lean burgers.
  • Save some calories for potassium and protein for after you work out, this will help reduce soreness. (A banana and an egg does the trick.)
  • Do not forget to stretch. Stretching can help increase your strength and flexibility.
  • Cut down on your sodium. Many prepared foods have as much as 30% of your daily value. You get your daily requirement of sodium from a healthy diet; too much can cause you to retain water weight.
  • Try not to skip any meals as this only makes your body to retain fats for energy. A good weight loss plan is to eat healthily and exercise regularly.
  • To continue to lose weight fast[5] - eat a variety of foods. Just like in weight training, the body will get used to the same routine and plateau. Mix up the foods you eat to keep your metabolism high and prevent a metabolism plateau.

  • Do not try to starve yourself. In the short run, reducing your calorie intake to unhealthily low levels will slow metabolism and actually cause you to lose weight more slowly. Then your body cannot process potassium so when you DO eat again, (the alternative to dying,) you will gain much more weight than you would if you weren"t starving yourself and overall, you may end up gaining more than you lost. In the long run, starving yourself (anorexia or bulimia) can kill you.
  • When creating a weight lifting routine, do not focus on a single muscle or group, doing so will overbuild that muscle while under building others - in other words, if you work your biceps, and not your triceps, your arms will begin to curl inward and you will look silly. If you work your chest and not your back, you will have a hard time maintaining good posture as your pectoral muscles overpower your back muscles and pull your shoulders forward.
  • If you have a serious medical condition (such as asthma or diabetes, or allergy to protein such as dairy products or you have lactose intolerance) or you are severely obese, consult with your doctor as well as a dietitian.
  • Do not take any harmful drugs. They are unhealthy and may ultimately make you gain weight instead.
  • Carefully consider whether you need dietary supplements. Most people may get the basic minerals and nutrients they need to sustain life through eating food. For example, excessive multivitamin supplements can make you overdose in some vitamins you need, and you can"t store most water-soluble vitamins like vitamin C as any excess will be urinated. Ever wonder what an average day is like for a Maternal-Fetal medicine fellow? I decided to keep a little log of my day today and post it on the blog. I decided that last night, and then I proceeded to have a day that is so odd (not bad.. just odd) that I"m wondering if I"m actually delusional or if all of these things are really happening. However, I didn"t change my day, because it didn"t seem like it would be true to my original intent. So.. enjoy.

5:15 am: First alarm. The exercise alarm. Decide not to exercise, and turn it off.

6:00 am: Second alarm. The real alarm. Snooze x 2.

6:20am: Awake to some commotion in my kitchen. Decide it"s probably just the cats. Roll over for a few minutes, hear animal screaming.

6:21 am: Arrive in kitchen to find the cats finishing killing a racoon. Yes that"s right. A racoon. A smallish racoon, but a racoon just the same. I always thought my cats were sweet, and kind of fat, and kind of lazy. I guess I forgot that they"re killers. And since when do housecats hunt in packs?

6:22 am: After deciding that this really was real, clean up racoon carcass. Brief moment of happiness when realizing that the cats rabies vaccinations are up to date.

6:30 am: Breakfast. Today it"s yogurt.

6:35am: Shower, get dressed, start the first 6 of the 18 pills a day I"m supposed to be taking right now.

7:00am: Leave for work.

7:30am: Arrive at the first hospital. A community hospital, to now be known as Random Community Hospital (or RCH) where I get paid to teach. A tidy moonlighting type gig that lasts for an hour in the morning a few times a month. Spend the next hour discussing patients at the residents" morning board signout rounds.

8:30am: Leave RCH for real job at Mothership Hospital (or MSH).

9:00 am: Arrive at MSH. Ponder going to grand rounds and realize that grand rounds is about global warming. They must have been desparate for speakers. Decide to skip.

9:20 am: Arive at the basic science lab I"m doing my project in. Chat with science geeks for a while.

10:00 am: Start a day long project isolating DNA bound to certain proteins.

11:30 am: Half hour incubation step in project. Work on paperwork and updating lab notebook.

12:15 pm: Second half hour incubation step. Eat lunch with science geeks.

1300: Third half hour incubation step. Continue quest to update lab notebook.

13:50: 3 hour incubation waiting period. Work on lab notebook more.

1400: Start 3 hour RNA isolation protocol.

1700: Start PCR to check the newly isolated RNA for DNA contamination.

1815: Finish DNA isolation.

1830: Medical student ob-gyn interest group mixer. Fun, conversation, all that.

2015: Mixer over. Stop at grocery store on the way home. Outside of grocery store is a man walking around with a live possum over his shoulder. Question my sanity.

2115: Call best friend to discuss very surreal day.

2135: Put groceries away. Wash dishes in sink. Make lunch for tomorrow, and get clothes laid out for the morning.

2215: Blogging!

2300 (estimated): Collapse into bed, read 15 minutes of "Middlesex" (I do at least 15 minutes of pleasure reading a day, every day unless I can"t keep my eyes open).


So, anyway, I guess that"s my day. Other than the rodent mischief, it"s a pretty normal day while I"m on a research month. I"ll be sure to do this again one day when I"m on a different service.

I have a new-found respect for waitresses and anyone else that spends all day on his or her feet. I wasn"t expecting my feet to hurt this much after spending all day in the lab. And getting off them doesn"t seem to help all that much, either. I think it just restores all the bloodflow to the areas that had been missing it; when they wakeup, they wake up angry.

Of course, wasting half a day in the lab because we couldn"t get a computer working right, doesn"t do much for my mood. It seems a software update had been installed that changed all the commands from Alt-(letter) to just (letter). Took us four hours to realize that. We"d rebooted the machine and had been looking for mechanical faults for some time when I foumd that out. You"d think that an important change like that might have made into -- oh, I don"t know -- the freaking instructions?

Skepticism: Where it is, Where it isn"t

Where it is: The lates issue of the Skeptic"s Circle, over at Left Brain/Right Brain. Read. Learn.

Where it isn"t: The campus newspaper, Imprint. Last week, it included an article which took the merits of organic foods for granted. I sent in a letter trying to set them straight. This week? Letter wasn"t published, and no word on the issue whatsoever.

UPDATE: Okay, they"ve now published it, a week later. Better late than never, I guess.

Specter"s spectre

There"s a big brouhaha growing in DC about Arlen Specter"s comments about what will happen when Bush II should nominate a pro-life judge for a federal or even Supreme Court position. He is now being attacked right and left, or rather Right and left, for being "disloyal." This increasing notion that Party loyalty trumps every other consideration is beginning to disturb me.

Arlen Specter wasn"t elected by his consituents to do what"s best for the Republican Party. He was elected to look after Pennsylvania"s best interests. And the idea that he and, presumably, every Republican is expected to rubber stamp any action by the President is dangerous.

It"s been said many times that politics is about compromise. Hundreds of Senators and Representatives come together from many different states, with many different competing interests, and try to hammer out aggreements and make laws that are good for the country as a whole. Not to implement Party doctrine. And, lately, it seems that the Republicans are treating this election with the attitude of, "Ha, ha, we can do whatever we want now."

Similarly, the CIA has begun a purge of those similarly "disloyal" to the president. The linked Houston Chronicle article reads in part:

"The agency is being purged on instructions from the White House," said a former senior CIA official who maintains close ties to the agency and to the White House. "Goss was given instructions ... to get rid of those soft leakers and liberal Democrats. The CIA is looked on by the White House as a hotbed of liberals and people who have been obstructing the president"s agenda."

Let me make this very clear: it is not the CIA"s job to assit the President"s agenda. It is the CIA"s job to provide intelligence to the government. Period. If the administration can not spin that intelligence to support their policies, that"s their problem. They have no business firing people because they might tell the Emperor he has no clothes. Dissent is not disloyalty.

These two things taken together bother me greatly. Now, I don"t say this lightly --- I am not a conspiracy theorist and I don"t want to sound shrill -- but I think these could be the baby steps toward an authoritarian state. We are seeing one branch of the goverment acting more and more to secure, not the ability to serve the electorate, but it"s own power. There is a fine line between no one being able to speak the truth for fear of reprisals to no one being permitted to speak without punishment.

Just a further note about the Specter situation: the right-wing press machine is already starting to spin him as a "liberal," and even an "extreme liberal." He"s not. He"s a moderate Republican. He is reasonably pro-choice and pro-stem-cell-research. The Christian Coalition gave him an 81% rating in December of 2003.

Acne - Dermatologists

Dermatologists are skin specialists who treat disorders of the skin. But they don"t just treat skin problems: they treat health problems of the hair and nail as wells. They are full-fledged medical doctors trained to diagnose and treat diseases and disorders of the skin, hair and nails.

The most common skin disease that dermatologists treat is acne. Acne is a skin condition that nearly all people have to contend with at some point during their teen years. The cases of acne vary from mild (the occasional pimple or zit) all the way to very severe (which consists of deep acne lesions and often extensive scarring). Acne is caused by the overproduction of oil from the hair follicles sebaceous glands, which combines with dead cells to clog pores. This can lead to whiteheads, blackheads, pustules and papules, or in the most severe cases, cysts.

Most acne cases don"t require the services of a dermatologist, as a healthy skin care regimen and over the counter medications can control it. But for those persistent, moderate to severe cases, a dermatologist is almost always needed. These doctors can prescribe stronger topical medications as well as oral antibiotics, to help get a handle on difficult cases. They can also provide the medical means by which acne scars are reduced or eliminated.

Dermatologists also diagnose and treat other skin disorders such as skin cancer. Doctors will examine suspected skin cancer sites on the body and if need be, perform a biopsy to verify cancer"s presence. There are a variety of types of skin cancer of varying degrees of seriousness. Needless to say, a doctor should examine any person who suspects that he or she may have some form of skin cancer, immediately.

Psoriasis is another skin ailment treated by these skin doctors. This is a chronic red scaly, sometimes itchy rash that affects a fairly large segment of the population. It can range in severity from barely noticeable, to needing hospitalization. Aside from the skin rash, a person may experience some degree of pains in various joints - which can be debilitating. It is thought that the immune system is involved in causing psoriasis, though this is not certain. There does seem to be a genetic predisposition to this disease. While there is no cure for it there are effective treatments that a dermatologist can provide after diagnosis.

Dermatologists also treat warts. These rough skin tumor-like growths can appear anywhere (but usually on the hands and feet), and are caused by viruses. Some warts go away by themselves but others can persist for many years. Dermatologists can treat warts in several ways. One option is to wait and see if it goes away on it"s own. Also, they can use salicylic acid in liquid form or patch. This often will gradually make the wart disappear. They can freeze it, gently, over a period of time. They can also employ surgery, lasers, and chemical cautery to get rid of them.

Dermatologists can also treat patients to remove scars. They can use lasers of varying types to reduce, or completely get rid of scars. The types of scars that can be treated are those that result from injury, acne scars, keloids, and hypertrophic scars. With today"s modern lasers, scarring can be reduced to a degree that it never could be before.

These are but a few of the many conditions dermatologists can treat. This is a field of many different specialties and areas of expertise and it is the state of the art in medicine.

Background
Research in homeopathy has traditionally addressed itself to defining the effectiveness of homeopathic potencies in comparison to placebo medication. There is now increasing awareness that the homeopathic consultation is in itself a therapeutic intervention working independently or synergistically with the prescribed remedy. Our objective was to identify and evalute potential active ingredients of the homeopathic approach in a prospective formal case series, which draws on actual consultation data, and is based on the MRC framework for the evaluation of complex interventions.

Methods
Following on from a theoretical review of how homeopathic care might mediate its effects, 18 patients were prospectively recruited to a case series based at Bristol Homeopathic Hospital. Patients, who lived with one of three index conditions, were interviewed before and after a five visit package of care. All consultations were recorded and transcribed verbatim. Additional data, including generic and condition-specific questionnaires, artwork and significant other reports were collected. Textual data was subject to thematic analysis and triangulated with other sources.

Results
We judged that around one third of patients had experienced a major improvement in their health over the study period, a third had some improvement and a third had no improvement. Putative active ingredients included the patients openness to the mind-body connection, consultational empathy, in-depth enquiry into bodily complaints, disclosure, the remedy matching process and, apparently, the homeopathic remedies themselves.

Conclusions
This study has has identified, using primary consultation and other data, a range of factors that might account for the effectiveness of homeopathic care. Some of these, such as empathy, are non-specific. Others, such as the remedy matching process, are specific to homepathy. These findings counsel against the use of placebo-controlled RCT designs in which both arms would potentially be receiving specific active ingredients. Future research in homeopathy should focus on pragmatic trials and seek to confirm or refute the therapeutic role of constructs such as patient openness, disclosure and homeopathicity.

A Tale of Two Births (Part 2/2)


So, the first post talked about one form of operative delivery, a low forceps assisted vaginal delivery. This post is going to be about a vacuum assisted vaginal delivery.

It was about 2:30pm. I"d been on call the night before, and I was just on the verge of being able to leave the hospital, after having been there since 6am the previous morning. I wandered to L&D to say bye to the attending of the day, and one of the midwives (my favorite midwife, actually, so we"ll call her Favorite Midwife, or just FM) came up to me and asked for my help with one of her patients. This patient was Spanish speaking only. In trying to deliver her first baby, she"d been pushing for 2 hours, and she"d become exhausted.

I go to assess her, and there was enough molding in the baby"s skull (the skull becomes deformed by the forces of labor and the pelvis. In this baby"s case, the sutures in the skull were overlapping each other, making it difficult to feel the baby"s position) that I couldn"t really tell the position with certainty. I tried to feel for an ear, but couldn"t quite get there. There did, however, seem to be adequate space to deliver the baby. I thought the baby was LOA with a significant asynclitism (baby"s head kind of turned off to one side). Because it was a private midwife patient, FM was there to assist me. There weren"t any residents available (it was a busy day on labor and delivery, and the residents were all in other deliveries or the c/s in the back) to do the vacuum, anyway, so I didn"t bother calling one to ask if they wanted to do the vacuum. Besides, it"s kind of nice to get to actually do a procedure I"m always letting the residents do anyway.

I discussed the options with the mom through the interpreters. The options were: 1) C/S now. 2) attempt at forceps (which I didn"t recommend because I"m not great at using forceps to correct an asynclitic head, although it certainly can be done) 3) Vacuum.

The patient agreed to a vacuum assisted delivery. The first vacuum I was handed was malfunctioning and could not hold any suction. I applied the second vacuum, waited for a contraction, and asked the patient to push as I started to pull. Some patients get to the point where they can"t focus, they can"t push because they"re too scared and too exhausted. The interpreters (also family members) were yelling. This was just too much. After about 5 minutes of trying to get this to work, the vacuum broke. It"s just a plastic thing and one of the plastic connections broke. But seeing the vacuum break made the mom totally freak out. The mom was yelling in Spanish. The family members/interpreters were yelling. The father was yelling, "I don"t care about the baby, just save my wife!" It was total chaos.

So I stopped.

I waited.

The baby was fine. The tracing was fine. There was no reason we couldn"t wait. But a vacuum assisted delivery requires more help from the mom than a forceps assisted delivery, and there was no way the mom was going to be able to cooperate while she was freaking out. I looked at FM and kind of sighed. She smiled and gave me a little hug. It gave me hope that I would get out of the delivery and out of the hospital eventually.

After probably 3 minutes of freaking out (although it seemed like 30), the mom had stopped yelling, the interpreters stopped yelling, and the father stopped yelling. I told everybody, "We are not going to be able to deliver this baby with all of this yelling. I need you all to know that everything"s going just fine. The baby is just fine, mom is just fine. But I need her to listen to me. I need everybody else to be quiet. Only one person should talk at a time, and we need everything to be calm for this to work. Otherwise, we should move towards a c-section now."

That seemed to quiet everybody down. With less chaos in the room, mom was much more able to cooperate and push when the contractions came and I pulled. After pulling the baby down a bit, I was better able to adjust the vacuum to correct for the asynclitic head, and the baby delivered without much further ado. She required an episiotomy, mostly just because of the operative delivery, but it was performed by FM and did not extend into the sphincter or the rectum. It should heal just fine.

The mom was very grateful afterwards, and the father gave me a hug. Really the mom did almost all of the work, I just gave her a little hand.

The thing is, never underestimate the value of calm. Calm is key. The previous delivery was easy because of the calm. This delivery was difficult because of the chaos.

First, I would like to point out that whoever scheduled a total of ten different examinations over the course of a three-week period needs to be informed that cruel and unusual punishment is prohibited by the Bill of Rights. I suspect that by the end of this week I will have lost what little sanity I have left.

But that"s not what I"m posting to say--complaining about examinations may be part of student life, but there are more entertaining remarks that I can make. So here are a couple funny highlights from recent work shifts.

A girl calling me for a refill on her oral contraceptive pills expressed sincere belief when I told her that we didn"t have the item in stock and that it would not come in until Thursday. The particular product recently went generic and there are still a few holdovers (despite the fact that they"re made by the same company), but either way supplies of the brand name are not as numerous as they once were on our shelves.

I believe her exact phrasing was "wow, you guys can run out of drugs?" No, silly me, let me go open up the extradimensional pharmaceutical holding chamber where we keep the infinite supply.

But why reference Michael Crichton?

Those of you who have seen the movie Jurassic Park may recall a particular scene in which the power is out and most of the main cast is sealed in one of the bunkers, hiding from the rampaging dinosaurs. Attempting to formulate a plan, game warden Robert Muldoon brings up "the lysine contingency," a totally ridiculous safeguard thereafter explained by Samuel L. Jackson"s computer programmer character:

"The lysine contingency - it"s intended to prevent the spread of the animals is case they ever got off the island. Dr. Wu inserted a gene that makes a single faulty enzyme in protein metabolism. The animals can"t manufacture the amino acid lysine. Unless they"re continually supplied with lysine by us, they"ll slip into a coma and die."

Of course, this makes absolutely no sense. Lysine is an essential amino acid, meaning that it cannot be synthesized by the human body--it must be consumed in the diet. In fact, no animals "manufacture" lysine. And as it is common enough--contained in many plants and all meat products--the "lysine contingency" isn"t much of a plan at all.

I mostly mention this because I had trouble refraining from laughing the other day when a woman was desperately searching for L-lysine supplements. I should"ve asked her if she needed it to keep her pet dinosaur alive.

Ugh, more traveling.

Back again. Sigh. The last for a while, I certainly hope.


But I came home to find a new, itty bitty fish in my tank. It looks like one of the plays had a baby. Possibly multiple babies. We won"t discuss what happened to the little guy"s brothers and sisters. Anyway, it"s cute. Pictures coming soon. Does this mean I"m a grandfather or just a fish pimp?

A new study by Harvard School of Public Health (HSPH) and Brigham and Women"s Hospital (BWH) researchers has found that, among a large study population of women who lived until at least age 70, being overweight in mid-life was associated with having more health problems later in life, including multiple chronic diseases, and impaired cognitive function, physical function and mental health. Why be burdened by highly publicized fears that decades later did not turn out to be true?

Human beings never get tired of discussing the latest report that tells us the end is near, it"s only convincing me that we might regard each new claim with skepticism, search information and make our own mind up.

He denies the global warming, he argues that scientific evidence for global warming is weak. Right now earth is clearly getting warmer, but I believe that the jury is still out. I am somewhat skeptical of global warming.

He ran to the bathroom sink and spewed a mouthful of blood.

Juarez's strain — so-called extremely drug-resistant (XXDR) TB — has never before been seen in the U.S., according to Dr. David Ashkin, one of the nation's leading experts on tuberculosis. XXDR tuberculosis is so rare that only a handful of other people in the world are thought to have had it.

Today, all the leading killer infectious diseases on the planet — TB, malaria and HIV among them — are mutating at an alarming rate, hitchhiking their way in and out of countries. The reason: Overuse and misuse of the very drugs that were supposed to save us.

"We are all connected by the air we breathe, and that is why this must be everyone's problem."

This April, the World Health Organization sounded alarms by holding its first drug-resistant TB conference in Beijing. The message was clear — the disease has already spread to all continents and is increasing rapidly. Even worse, WHO estimates only 1 percent of resistant patients received appropriate treatment last year.

"We have seen a huge up burst in resistance," said CDC epidemiologist Dr. Laurie Hicks.
